I did google this but couldn't find any usefull information. I want to create a paper petition for an international sports tradeshow that can effect a specific sport worldwide. My specific question is: What information is usually looked for next to signatures? I know that sometimes they ask for DL# or SSN but I want to make sure that we don't ask for too much info but give the international party we are giving the petition to enough.
 
Just ask for name, city, state, zip. Don't ask for anything more personal. No matter how much info you collect, they'll either believe it's legitimate or they won't.
