Personal statement for college application... how do you format it?

Do you sign it? Is there supposed to be a title? I've searched a bit and all the sites have tips on how to write one or what to say in each paragraph, but no info on those details. Any help?

I titled mine, and put my name and social security number in the top corner. That's generally the format you should follow (in case your papers get misplaced). A title isn't necessary, but it doesn't hurt.
