So allegedly, the group insurance rates that my employer is paying are going up. They want to shop around for better rates.

After filling it out I'm supposed to fax it to their insurance agent who will then send it out to any company that we want to get rate estimates from.
I'm really hestitant to fill this out and fax it to some company and have no knowledge of where it's being sent. It contains my SSN, DOB, and complete medical history.
My employer claims that if I don't, they can't get a quote from the insurer. My only other alternative is to waive insurance and then I could enroll after some waiting period.
Is this a normal procedure? Seems absolutely ridiculous to me that I have to authorize full disclosure of medical information to private parties not bound by privacy laws and give them the right to obtain and disclose medical records just so we can get an estimate of insurance rates.
