Yes, you have to fill out the forms, sign them in front of the clerk with valid ID's (I think you may need a birth certificate or naturalization papers), have the correct format pictures to go with them, etc. I think it takes 2 to 4 weeks to get them back. You can get all the forms and instructions on line at the Passport Office website. A tip - the passports are good for 10 years now. If you expect to do any reasonable amount of traveling, especially to places that require visas, get supplemental pages added when it is first issued. (There is a form for this too.) This saves having to send it back in after a few years to get more pages. I just realized mine is almost full after less than 4 years and I've got to get the added pages put in quickly.
