Passport application question

So, I've got this passport application, & am ready to turn it in, but question 17 " Have you ever applied or been issued a US passport." It has a space for Most recent passport number and asks you to submit your last. I got one when I was 14 or 15 (11 or 12 years ago) but I lost it a few years ago.

What should I do? Should I just say no because it was so long ago?

if you lost your passport, i think you have to apply to get it replaced, not apply for a new one

isn't there a form on the site for passport replacement?

Your previous passport has expired and it was issued when you were under 16. Yes
Your name has changed since your passport was issued and you do not have a legal document formally changing your name. Yes

so that says you have to apply for renewal if you lost your passport

dont worry about it i had the same issue called them, they said that i wasent even in they system, in fact no one who was under 18 10 years ago was put into any system

which is why i cant renew mine online i have to go to the PO and all that

Awesome, ten years ago I was 16 and had the passport for 1 or 2 years at that point. Thanks.

so that says you have to apply for renewal if you lost your passport

He didn't lose it, it expired...THEN he lost it.

I bolded the situation that applies.
