Also, the packing slip has a order number in the upper righthand corner of the page. If you go to the staples.com website, and select "Order Status", you should be able to enter this order number, and view an invoice for this order with the 1/19/01 purchase date, which you can then print out.

Unfortunately my shipment came from a Staples store, and they don't actually charge the credit card until they arejust about to ship the item, just as if I was shopping at the store. Unfortunately they did not ship until tuesday, so I don't have that 1/19/00 date any where. Fortunately like people are saying here, my order confirmation number takes me to a page with the order date of 1/19/00 and shipping date of 1/22/00. I talked to Palm about this and they said it should be okay, and just send the forms in. I guess I just have to hope for the best.
