Overvoltage alarm when I plugged in firewire card reader

I've had a newly built spare computer sitting for the last several months and this evening I decided to put it into service. The computer was built and configured several months ago without a hitch or hiccup. The firewire PCI card was installed when built and is recognized in my Device Manager with no conflicts.
This evening I plugged (first use of the firewire port) a Lexar Flashcard reader in and got an instant hardware monitor alarm. The malfunction was high voltage on the +12 volt line. My high limit on that was set to 13.1 volts so it surged past that for sure. Since I run a front panel acces kit on this computer I decided that the first step would be to eliminate it (the front acces) so I plugged the reader directly into the port on the firewire card .. same thing.
Since the firewire card has been in there and is recognized I have to assume that the card reader is junk. Opinions?
