Overclocking woes

About a month ago I built my new rig, the specs of which are:
i5 2500k @ 4.0GHz
Corsair H100 Liquid Cooler
ASrock Extreme3 Gen 3 Mobo
Hitachi Deskstar 1000K.D 750 GB
Intel 320 Series 120 GB SSD
GTX 460
16 GB Corsair Vengeance RAM @ 1600 MHz

At first, I was able to get my processor to a 4.5 GHz stable overclock, but then my motherboard started having problems with the RAM -- only using the third and fourth slots would allow the computer to POST (thus dual-channel configuration can't be utilized), so I RMA'd the mobo. I finally got around to putting it back together last night and everything works fine, except I'm only able to get a pretty modest overclock considering how much I spent on cooling. This is my first try at overclocking, so I'm sure I'm doing plenty of things incorrectly, but even when I do the built-in auto overclock on the motherboard, I can't even get 4.2 GHz to be stable. One or two Prime 95 workers fail after about a half hour of running, but Intel Burn Test passes on standard (do I need to be running this at maximum?) When I ran the Intel Burn Test on High, I set the number of runs to 10 and it passed them all, but right after I got the message saying it had successfully completed the test, I got BSOD.

Right now I'm running at a multiplier of 40 with 100 MHz base, max current allowed in processor upped from 150 to 250, LLC set at Level 4, and voltage at auto. Previously I had the voltage set to 1.37 or so, with plans to decrease it once I found a stable number for that voltage. If you need any more information I'll do my best to provide it. I'm having woes too. My 3.8GHz OC is no longer stable. Of course my i5-750 only cost me $100 and my cooler only $5. So I just settled on 3.5GHz for now. If I had the setup you had I would be irritated. Can you return the Corsair H100? It's most likely memory, but it takes time to find the right timings and at some point you just wanna say heck with it...
 
I'm pretty sure I'm out of the return period for the H100 by now. The only thing that's changed since the configuration is the motherboard and the addition of two more sticks of RAM. I hope it's not the memory like you said, as that does indeed sound like it would involve a very tedious tuning process...
 
I settled at a 41x overclock @ 100.2 bus, I just couldn't get mine over 4.20GHz reliabily without overheating it, or getting a failure in LinX testing.

I'm going to take a wild guess and say you are going to have to adjust your vCore manually, that's what was causing my CPU heat problems, and I don't really think it needs all that voltage (mine didn't.) I would probably go down to 2 sticks of RAM and try it again.
 
Updated to V 1.3 and it still fails Prime 95 after 1/2 hr to 1 hr, but passes Intel Burn test on Standard, High, and Very High. Haven't tried max yet.
 
I'm running the processor on completely default settings now, and Prime95 fails after 45 minutes. What's going on???
 
You're right. It is a memory issue. Memtest comes up with errors when it runs. Looks like it's time to buy some new RAM... Thanks for the help
