Outlook: Getting my format toolbar back...

I have no idea how the formatting toolbar "disappeared" a few days ago at work... Now, each time want to bold or color text, I have to bring up the menu manually. I've tried all the obvious menu picks for toolbars, formatting, etc... I've used "HELP" and haven't found anything there about getting the formatting toolbar back by default when I compose an original email, reply or forward.

HOW do I get this toolbar back by default?
 
Standard, Advanced, Menu Bar, Remote, and Web are my only box choices to check... There is no "Format" Box although I think there used to be. Perhaps I deleted it by mistake (from this dialog box). How can I get it back?
 
Which version of Outlook and which editor being used (internal Outlook editor, Word, etc)?

In using Outlook 2003 with Word 2003 set as the editor, the toolbar being sought is found under Toolbars>Email.
