Outlook forms

Our HR manager is trying to create a pretty simple survey using outlook for personnel evaluations. She wants people to be able to see the form via outlook, enter information (in text boxes, check boxes, etc) then click send and have the information come back to her looking the same as the way they sent it. Probably some kind of 3rd party program that automatically generates outlook forms would work. Any suggestions?

I haven't used them firsthand, but we had a few developers experiement with them... according to them, it was WAY more trouble than it was worth. A quick and dirty web app will be much easier to setup and use.
