On an application, what does "Familiar Name" mean?

It lists a field for Given Name and Surname and also Familiar name.

It says in brackets "to be used on name cards, etc", so I just assume it would be my first and last name without my middle initials?

I would guess the name you go by. Say you have some complicated came and you simplify it a bit. I guess that would make sense being the name people are "familiar" with calling you.

Your resume says your name is William or Robert.

Your "familliar name" might be Bill or Bob. Or maybe you use your middle name most of the time...

I go by my middle name. I would put my middle name in that spot.

Another way to put it would be "preferred name"
