Odd request: OCZ Powerstream 520W *Rebate Form*

Does anyone have it? I totally blanked and forgot to print it out when I ordered, partially because I was expecting a physical copy of the rebate form with the shipment, but none came. Now the deal is gone and the rebate was pulled 🙁

I don't suppose anyone would happen to have it still, or know where I could get it? I've already e-mailed Monarch, but haven't gotten a reply.

Try going to their forums and looking around. Same thing happened to me with some OCZ ram. Nice PSU btw.
 
Login to Monarch forums (pre-sales forum should be able to help) and ask there. Usually a monarch rep browsing the forums.
