Odd 6950 Graphical Issue

I bought a Sapphire Radeon HD 6950 2GB this past summer and it's been running everything perfectly well since. Unfortunately, during the last couple of weeks I've been getting an on and off graphical issue. It has happened while playing Skyrim, ARMA II, and Battlefield 3. When I'm playing the game, It'll launch fine and play fine for about 10 minutes, then suddenly, typically the right third of the desktop will sudden swap to the far left side of the desktop. It makes the picture loop around. I do have a dual monitor setup and I can still move the mouse between the 2 monitors. Even when I minimize the game, the split is still there. I have to disable the main monitor and re-enable it in windows to finally fix issue. The temps and clocks on the GPU are normal, nothing extreme. Drivers are up to date and I have tried using just one monitor, the issue still occurs. It doesn't happen every day, some days I can play for hours without it happening. I'm not sure what to call this problem, cause I'm not sure if it'd be considered "tearing". Any help would be great guys, thanks!
