I mentioned on this thread some problems I had with an older (7 series) Nvidia graphics card connected via DVI on Win8 and Vista:

I've since seen a machine with an 8-series Nvidia card with an extra monitor connected via DVI also lose its DVI connection, and it was due to the same driver version which was pushed as an 'important' automatic update on Vista (so it goes on completely automatically unlike most driver updates via Windows Update).

It is labelled as a WDDM 1.2 Nvidia driver update (I didn't note down the KB number, sorry). It may only affect DVI users (no idea about HDMI), as the second machine with the 8-series card had two monitors connected, one via VGA and the other via DVI, and the VGA connection was fine.

The nvidia driver version is 306.97, and the problem exists with the official driver release from nvidia as well.

I don't know how many generations of nvidia cards this issue might affect, but considering that it affects users running Vista and Win8, I suspect Win7 users might also experience the problem.
