No dayrunner for me, I want a 3-ring binder, but....

Hello,
After looking at dozens of personal organizer (paper one's) I have decided that none of them work for me and I will be creating a custom organizer with a simple 3 ring binder. It seems like some of the best come from www.bindertek.com. So I now have 2 questions for your all-knowing internet minds:
1. Do you know of any high-end 3 ring binder companies?
2. Do you know of any sites where I can download forms (calender, phone contacts, personal notes etc) the be used in my 3-ring binder?
