newegg.com rebates

Yesterday I bought an ati x1900xtx 512mb from newegg.com for about $375. It has a $30 mail in rebate. Im not sure what I must submit for the rebate. When I clicked on the rebate form link at newegg.com site. It popped up the mail in re-bate for for my video card. At the top it states here is how you get your rebate:

1) Fill out this form completely. Incomplete forms will be disqualified.
2) Must go to www.MyRebates411.com, login, fill out the required information,
and submit a new rebate request
3) Fill out this form and mail it with the following proof of purchase of the specific
product(s) to the address below:
? Original or copy of invoice downloaded from Newegg?s website.
? Must write Newegg.com, its Item Number, the promo code, and
MyRebates411 Transaction ID on the back bottom of the envelope.
? Original UPC barcode including Serial number
cut from retail box. Label must include serial number
or customer will be disqualified

I'm not sure If I have to do all 3 of them, or just goingto www.myrebates411.com will work just fine. Has anyone else delt with rebates through newegg recently?
 
And it will probably take at least 12-14 weeks for it to arrive, at least the last few newegg rebates I received did.
