Newb question!!! about a rebate

I bought a hard drive from frys, and was wondering if there's a form for the rebate. All i got was a copy of the reciept that the register printed up with a place for my address at the bottom. It's a 80gb western digital hard drive with a $40 mail in rebate. Please help!!
