The area of these wounds swells up, The bones in the pic look bigger because the arm is close to the camera and the body is further away.
Towards the end they doctors are working on a woman with a healed wound that has the bone sticking out and seems to be the patient that this photo was taken of.
Not to say Anyone is wrong by judging from the pic, but the effects of this drug ravage the body and can make it hard to tell WTF you are really looking at.
