new network, new acceptable use policy?

I am creating a new network for a nursing home, from the wire plant on up.
I advised the director that they need a policy specifically for computer usage.
Do you have any suggestions for a format? I googled and found a few examples, but thought you might have a nice generic one that can be copied and used legally.
Also, what is the best way to disable internet browsing for a regular user?
I'd hate to have to go to that extreme, but I'd do it right now if it meant preventing patient neglect.
