I have had this card for a little less than a month. This is a new issue that has only happened once but it's still cause for concern. I loaded a youtube video and the entire screen filled up with horizontal bars. The strange thing is that the computer was still up and functioning. I still heard the music. I hit reset and it is working fine now.

Is this a driver issue or should I look into warranty?

I had an similar problem with a 7950, and it's cause for RMA.

I never figured out exactly what it was, but I suspected something defective in the card's memory due to the way the horizontal bars resembled the brief flickering that appears on a secondary monitor when the memory speed changes.
 
I hope it's not a memory thing. I will keep an eye out and try to replicate it again.

I had driver 12.10 before but 12.11 didn't crash until I used it for around three weeks.
