Need some help locating replacement chokes for my HD 4850 :(

The chokes on my card were ripped off when I attempted to remove a epoxied RAMsink. Needless to say it doesn't work without that component. It's the grey part near the rear of the card that can be seen here. Anyone know what the exact model number is, or where I could get replacements? Thanks.
