You should be able to pick up a Jars of Clay songbook at any Christian bookstore. I dunno if they'd have the sheet music for just that song though. It's notoriously hard to find sheet music for anything unless you're willing to buy it.
 
how does it go? hmm... downloading now to listen. but can't you just listen to the song and work out the chords yourself? i assume you're talking about guitar chords? ask a better guitar player and they will probably be able to work it out easily.
 
I've got the music for it and if you really want, I'll photo copy it and mail it to you. Just let me know.
