I've picked out my hardware components listed below:
a) Ryzen 5 2400G Processor with Radeon RX Vega 11 Graphics
b) ASRock Mini-ITX Motherboard - AB350 GAMING-ITX/AC
c) G.Skill Ripjaws V 16GB (2x 8 GB) (Dual Channel Kit) 3000MHz DDR4 288-Pin DDR4 SDRAM
d) Reusing an extra Samsung 1TB SDD
e) Reusing a 4TB WesternDigital 7200RPM hard drive
f) Reusing an ASUS DVD player/writer
g) Reusing a Microsoft Gaming keyboard
h) Reusing a SeaSonic M12II 620 Bronze 620W ATX1Ready 80 PLUS BRONZE Certified Full Modular Active PFC Power Supply

Now what I'm looking for in a small mini-ITX case:
a) Standard cooling (not looking for water cooled)
b) There will never be a graphics card installed, so space for one is not needed
c) Would prefer the power supply be mounted on top, above the motherboard so the heat will not effect the board or processor as much.
d) Would like to still used the DVD player/writer
e) If possible, I would like a front I/O panel for access to HD Audio ports, a SDHC 2.0 Card Reader and USB 3.0 ports

Any suggestions would be greatly appreciated.

Since you're trying to cram an ATX-sized PSU in there, you're going to find that most ITX cases which accomodate full-sized PSUs also have room for GPUs. (The assumption being that you wouldn't need more than a ~300w SFX PSU otherwise.) But at the same time, most of these cases use slim-style optical drives, so you won't be able to reuse the DVD-RW from your old rig.

I have an older version of this case and am pretty happy with it: http://www.silverstonetek.com/product.php?pid=484&area=en

If you really HAVE to reuse the 5.25" drive, then you're probably going to end up with something a little larger, like the always-popular Cooler Master Elite 130: https://www.newegg.com/Product/Product.aspx?Item=N82E16811119286

But again - they're assuming that if you want to use full-sized desktop components, you probably want a GPU, so again, they're building the case around that assumption.

If you are willing to replace both your PSU and Optical drive, the Silverstone Milo is pretty nice: http://www.silverstonetek.com/product.php?pid=653&area=en

Are you looking to stay within a particular budget for this case or just trying to get something that meets all/most of your requirements regardless of cost?

If you went with a slim dvd drive and SFX PSU, the NCase M1 would fit a lot of your needs (basically everything but the card reader). You could also get a USB 3.0 external drive and card reader and go really small with like a Dan Case A4.

These solutions are all on the spendy side, but are very nice IMO.

I’m seriously thinking about going a completely different route and doing a custom build. I’m looking for a cheap case in which i can cut the chassis apart and use inside a vintage radio case. So I will be going to a slim DVD drive and small and quiet power supply. I’ve got all the measurements recorded for the various components, and I’m going through thrift shops and antique stores looking for a non working radio with a case that needs refinishing.

I have a Corsair 380T that I really like. Its not the smallest ITX case, but cools well and has a full size PSU. And the PSU mounts upside down so that the fan faces out the bottom through a filter.

EDIT: totally missed your last post about wanting full custom :)
