i think the public notary is the one who should fill those one you turn in the form.

You dont fill that out......the Notary does.......and dont sign it until you are in front of the notary, they need to witness your signature.

just take it over there and do it there. you could fill out parts of it. just dont sign anything unless the dude is watching you, signs it and embosses the seal
