It's a timing crystal. A 12.000 MHz timing crystal, to be precise.

No idea where to get a replacement of that frequency in that size though - I suppose you could put a bigger one in, but the standard size for crystals is pretty big (about as big as that alcor micro chip).

The capacitor only has "12.000" written on it, I'm thinking volatage?

The Memory chip is Infineon HYF33DS512800ATC, G1, 0510, SUU10124

the back shows Alcor Micro, I'm assuming a memory controller. "AU9384B23-MAL", A36615-0, 0509.

Anyone tell me what kind of capacitor I need to fix this, I would just solder it back on but the lead wires broke off on the capacitor and my soldering isn't good enough for that small contact. I know its only 64 MB, but its holds a place in my heart...
Try running 12 volts through that thing and see what happens.

should any type work? I think the one in the pic is an "AT Strip" type from my research. Is there a way to tell if it is series or paralell of 20 pF or what have you? That is just providing a clock source so it can synchronize with the computer right?
 
That crystal drives the Alcor USB Controller. You can check for crystal input specs here to see what the critical parameters are.
