You can actually survive without it, but yea, ifixit and powerbookmedic are reputable, and there is always ebay. It is a bit of bent thin aluminum and some screws, there isn't a whole lot to screw up, and the ebay route would be cheapest.

Not really. How much is the MacBook moved around anymore? If it is still in a lot of traveling use, then you might want to get the cage, at the very least it should reduce movement, which isn't for the drive's sake, it has a Sudden Motion Sensor, but for the connector's sake, too much movement could break it.
