Anyone know what a "floppy disk(s) fail (40)" is? How do I fix it?

Tried old floppy, new one, old cables, new cables... still getting the same error.
 
Check all your floppy and IDE connectors first. It's likely that your floppy cable is knocked lose or hooked up incorrectly with that error.
