I just need a simple template for ms access 2000, for password management.

Just want to be able to input a persons name, username, and password. not allow duplicate entries and be able to generate a printable report from there. I thought this would be able to find but i cant find anything. Access really kinda confuses me, so building it from scratch would take forever. I would prefer not to use 3rd party software, and yes i know i could just do this in Excel, but like i said i want something that doesnt allow duplicate entries. any idea where i can find something like this?
