I had the same problem with my headphones for my Sony Walkman...remember those...they're still being made.

After looking around the 'net; I decided it wasn't worth the time and trouble; I found one piece in a drawer, but it was old and stiff and was too difficult to attach.

So I gave up and bought a new headset at Circuit City for about $ 15. I think and they're foldable, too.

Sony's website has plenty of info on the different models; if that's the brand you want. I can't wear the "in the ear" type and don't like the neck wrap type, so I chose the over the head model. YMMV

Koss sells inexpensive replacement pads for all their models (I believe).

I even paid $5 shipped for replacements for my cheap KTX Pro headphones ($15ish). Unfortunately, the pads they sent weren't the original OEM pads, which I later discovered are permanently hooked on. I could tear them out, but there's no way to adequately affix the replacement pads. Not to mention they are much inferior to the original pads (in fact they're thinner than my WORN-OUT pads).

I never bitched to Koss about these problems, but needless to say I'll hesitate to buy replacement pads for budget headphones from now on (as an aside, I was quite pleased with the sound of those headphones).
