Need a SCA-80 to 68 pin LVD Adapter + Terminator Connector combo. Buy where, please?

Made the mistake of buying a 80 pin SCA-80 UW2 SCSI HDD. Please, anyone know a reliable vendor for the LVD Adapter AND Terminator(1 unit) so that it'll hook up to a UW2 SCSI Controller with a 68 pin connector on it? Thanks!
