Need a new desk chair.

I need a new desk chair. It must have:

Adjustable arm rests
Adjustable height back (NOT just a seat height adjustment)
Head rest (Edit: I mean either a head rest, or a back high enough to rest your head against.)

I have $300 to spend. Costco has already failed me.
