Looking for something that help build surveys that can be published to an existing internal or external site but free if possible and compatible with IIS

Webmonkey looks like it'd be useful if I already knew what I was doing, but I'm a total noob with web anything.

Doesn't even have to be web really, just need a free solution on how to get to users a survey that they can fill out and I can get the data in a centralized form (xls/database etc..).

I believe its called C#2005.net + SQL Server 2005.net... and its free!

all you have to do is learn how to program it.

Users? Maybe 100 over a spread out period of time.

Simple question survey, like

1: was the person you spoke with helpful?
a yes
b no
c Not sure

***above is just a real rough idea of something I'd be looking for.

Main thing is, I need to be able to send to customers electronically, have them complete it and it be submitted back and added to a centralized database (or spread sheet) of sorts, automatically would be even better.

We've used surveymonkey (.com, google it) before... but I don't think it's free. Maybe they have some sort of free trial you can check out.

I was just looking at that, it's not free but looks reasonable.

Might be a second option if I can't at all find something I can host from my own site for free.

Seems like there would be a way to create a survey/poll whatever in outlook, email it out to anyone I want, have them fill it out and have the results submitted back to me in a way that I can automatically or manually add that data to a database.

Seems that way but there's not. Do you have shared drives? You could put together a quick form in Access.
