Need a fax cover sheet.....

I have to fax my resume over to a company tomorrow and need a professional fax cover sheet. I have searched on the web and can't find one.
Can anyone help?

do you have a real cover letter for the company? showing your interest in the company and why you would like to work there (and how it would benefit them, and don't be an ass about it)? a good cover letter can make the difference between your resume being read and it just being shoved in the pile with the rest of them. i would not use a form. of course, you should send off a top sheet giving your callback number and how many sheets are being faxed over, that can be a form.
