Need a bluetooth headset for a loud car.

My car is loud and not very well insulated from road noise, and neither are Starbucks with their coffee grinders, conversation, and music.

So it's hard for me to:

1. Hear the other person - I really prefer in-ear-monitor-style headphones for the noise isolation and ability to hear in noisy environments

2. Have the other end hear me clearly.

Any recommendations? I had a Blue Ant Z9i but it's battery is pretty much shot, and it's hard to hear anyway.

please don't use noise isolating in-ears while driving...

Just in one ear? How about in a noisy Starbucks? Next to a noisy road? Right now if I'm driving I have to take one hand off the wheel and press my headset hard and flush against my ear to hear things clearly. That's not safe. Or is it just that there's no way to safely make a phone call in a noisy car? I have to upgrade to at least a Camry level car to make calls safely? And then the car has better sound insulation, which means I won't be able to hear what's going on outside, anyway.

I have a Plantronics 975 and my car is very loud with the road noise from the tires to my exhaust and turbo, the quality of the speaker in the earpiece is awesome.
