Mylex BT950R - Can't load RAIDPlus Image?

I just got a BT950R, and it's behaving badly. I don't have an 'original' Mylex RAIDPlus disk (I bought all my 950R's and 952R's OEM) but I have the proper image on a floppy. In ONE system, attempting to enter RAIDPlus setup w/Ctrl-B at boot brings up the insert upgrade disk prompt. I do so, it starts loading the image, and the system locks up completely. No IRQ conflicts or anything. System is KT7-RAID, TBird 750, SB AWE64, Netgear FA311, GF2MX400, 256MB PC133 ECC. I've never had this problem with any of my other 950R's or 952R's. Suggestions? Ideas?
