New purchase. The card is 3Gb Windforce OC:
https://www.gigabyte.com/Graphics-Card/GV-N1060WF2OC-3GD#kf

This is the display: Sceptre U505CV-U
http://www.sceptre.com/overview-371.html

I've used other cards at 3840x2160 @ 60Hz successfully (GT 640, GTX 750 TI, another maker's "refurbished" 1060 that was defective and returned already). This Gigabyte one only shows a blank screen if I choose 60Hz. 4K 30Hz works, anything above won't. Using same port the others work on (rated for 4k 60Hz, plugged directly into the display) and same HDMI cable though I purchased a new HDMI cable to try that didn't make a difference. Also tried displayport out - no difference.

I've contacted Gigabyte and waiting for a response, but figured someone else might know what to do. If they provide a working solution I'll post back here

when you bought a new hdmi cable did you get a hdmi 2.0 compatible one?

those 2 older cards only do 4k60 with limited color space, maybe the problem is the new card trying to output the full color space over a cable that cant handle it.

I don't have the cable in front of me now but it was a GE brand advertised as supporting 4K & HDR

Turns out it was the cable after all. The one I originally purchased was actually Blackweb 12ft HDMI which is Wal Mart's house brand https://www.walmart.com/ip/Blackweb-4K-HDMI-Cable-12-Feet/55455424
which turns out to be the poorest rated cable I've ever seen. It wasn't the cheapest on the shelf (that would be ONN) but somewhat cheaper then the next highest up brand: GE. I figured why not save a little - after all the Blackweb is "Premium certified Cable" and the attractive packaging lists the following features: 4k, HDR, 18Gbps, 60hz

The replacement I bought today was GE Premium HDMI15 ft: https://www.walmart.com/ip/GE-15ft-Premium-HDMI-Cable-with-Ethernet/297729983#read-more
So far seems to be working great. Also has a lifetime warranty. Just say no to Blackweb
