I remember this one pretty well:

"n 1988, cracks caused part of the roof of an Aloha Airlines Boeing 737 to peel open in flight. A flight attendant plunged to her death."

I remember the Aloha airlines one. It happened 2 days after we had flown on one of their flights between islands.
