What about a trackball? The rest of the arm stays stationary, you just use your thumb to move the cursor.

Maybe the Trackball would be a better solution. I don't have a third hand. LOL Wish I did though as much as I am on the Computer.

Do I want to know what you are doing with said third hand? :)

My sister has rampant carpal tunnel, she used trackballs for the longest time. She found that the trackpad on her laptop was pretty easy on her as well.
