The "cyberdemon" (not Sarge) at the end of the game. Instead of getting a remake of a classic Doom enemy, we get a big pit, a bunch of Imps (as if we hadn't had enough of those during the game), and a creature that can only be hurt by the soul-cube instead of going at it with rocket launchers, chainguns, and for the unlucky, the crappy pistol. I.E. in this case fiendish = suck.
 
Balron in ultima online. First time I encountered it, it boinked my two pets, nightmare (magical horsie) and white wyrm (dragon) in less than 20 seconds.

Also Medusa heads in the castlevania series.
 
Mind Flayers. Early on one of those bastards can take out your entire party if you're unprepared. You pretty much have to lure them one at a time to beat them. Beholders and Demi-Liches both suck as well. Baldur's Gate II is filled with annoying enemies, actually.
