Modem driver

I have this modem and no driver for it it is not recognized when I load win98 makings:
modem blaster

Has a plug for a phon speaker amd mic.
I would like to use it but can't find a driver.
Any help appreciated

.....is it possible that this d15630 is supposed to be DI5630?
if so here.........link
using the FCC # GX5, I was able to find out that Digicom makes that modem, and doing a search I can to Creative.....so I came to that modem on creative's site and found

Modem Blaster Di5630 PCI modem.....hope that works.
