Hasn't been a thread on this in a while, so thought I'd bring it up for those who don't know or think this service is discontinued.
MS Ergo 4000 died on me last night, dug around for the Supplemental Parts phone number this morning and called in. Replacement keyboard should be here in 3-10 days.
You just need to describe the situation and give them the PID of the part you need replaced, no faxing of the bottom of the mouse/keyboard whatever.
