Can someone provide a quick overview of the benefits of Software Assurance? I've tried searching for it, but not everything is obvious.

Is the extra cost of a software assurance a one time fee?
Is there an annual fee?
What do you get for the extra cost? Extra service? Ability to upgrade to new versions of the program?
