Wish I still had a blank form for you, wjsulliv. I believe I might have a scan of the one I submitted with my handwriting all over it.

My status moved from processing to check approved sometime during the past 2 days. They still haven't mailed it.

call them. i got a bogus rejection due to it "not being mailed in by the postmark."
i told them that I did over the phone, and the guy pushned it through, no hassle.
this was abou a month ago.
i got the rebate last week :)

Is there a setting of full in the aspect? that should do it but it will stretch the image.
I personally don't mind it and after a few weeks you get used to it. But I personally like watching HDTV on me 50" plasma. And rarely watch standard def shows anymore.
