Question micro stutters or low 99th% fps in windowed borderless but not full screen

I built a new system recently and troubleshooting this one issue in my main game ffxiv has been driving me crazy. I game at 4k 60fps and cap my framerate in game to 60hz to match my tv. Much to my chagrin, ffxiv marries the fps limiter setting to vsync and removing the framerate cap in game removes the only functioning v sync.

When I game in windowed borderless in Windows 11 on my new machine my fps is 60 but runs like it's half of that. Sure enough, the 99th% fps seems to be locked to a perpetual 32fps that never goes up or down more than 1.

When I disabled optimizations for windowed games in windows graphics settings the 99th% fps problem gets fixed and it hovers around 58-60fps which I think is ideal, but while it no longer looks like it's running in 30fps it has a strange microstutter. Like the frame get stuck for a split second every second before catching up. This is most evident when moving slowly side to side in game which drives me crazy.

When I uncap the fps in game but cap it to 60 in AMD Adrenalin universal driver settings it removes the microstutter and the 99th% 32fps issue both, but introduces screen tearing that neither the driver's Enhanced Sync setting or "wait for vertical sync" setting does anything to fix. only FFXIV's in game frame rate limiter seems to sport a Vsync capable of preventing screen tearing.

When I limit the fps to 60 in game but set it to full screen the game is buttery smooth and has no low 99th% fps, micro stutter or screen tearing issue... Perfect! But, I hate gaming in full screen... Which I can't stand because I alt tab constantly and windows always has a heartattack every time I do. I've had long history of games crashing and getting stuck on top and becoming impossible to close by running games in full screen as well, so I really do need a fix for windowed borderless mode.

I had this same video card on my windows 10 system I just replaced and this wasn't an issue. What in blazes could be the problem?
