I guess you watched the short trailer? The long one has an explanation of Snake's condition. I thought Otacon and Raiden looked 5-10 years older than the "Big Shell" incident (MGS2).
 
Also Big Boss is in it...

The grave where Snake was saluting was The Boss' grave, that scene was really depressing at the end of MGS3. 🙁

No, I watched both, but couldn't really keep up with the blurry text in the subtitles. And I don't speak Japanese 🙂. So he has some sort of aging condition? Probably something to do with his genetic makeup? I see Liquid is still trapped inside Ocelot. This is gonna be weird, meeting these characters again after 2 games without them. Well... you know what I mean.

Ooh, maybe we get to see Snake kill him! At least some visual backstory.
 
I think Foxdie is out to kill him... it was only a matter of time.

Remember that injection he took in the other trailer... I bet its to stop the effects

Otacon had greying hair in MGS2, why is his hair dark brown again?

In the long trailer, Naomi tells Snake that his body is breaking down, which hints at what's going on. It could be FOXDIE as ballmode suggests, or it could be really old cloning technology.

It could be the harsh lighting of the scene in which he appears. I don't remember him having any gray hair, but I recall that he had sort of a baby-face, which could be misremembering due to his constant "wah wah E.E. wah wah."

I haven't played 3 yet, but I'm going to pick it up soon since the re-release Subsistence came out. From what I've read, you play as Big Boss much earlier in the chronology (he looks more or less like Solid Snake in MGS 1 and 2).

I started writing a plot synopsis of MGS2, but you'll be able to find something better on the web. 2 introduces Raiden, a soldier trained entirely in VR on his first real mission. He and Snake work together to stop the terrorists, disable Arsenal Gear, destroy several new Metal Gear Rays, and defeat Solidus Snake, another clone of Big Boss. Liquid Snake is revealed to be doing a time-share in Revolver Ocelot's body due to his arm being transplanted.
