dial 0 and ask them for help.

EDIT: and I'm serious. The operator can help you a helluva lost faster than we can.

Dial one number, get connected, hit flash, wait for dialtone, dial other number, get connected, hit flash again. Same procedure as transferring a call in most offices (except you don't hang up when you're done)

And if your phone doesn't have a flash button, hitting the hangup button usually does it (don't keep it down too long, though)
