Max Payne 3 Crashing, Freezing, BSOD, Glitches (Screenshot inside)

UPDATE 2: So I ran memtest 86 and started getting massive errors reporting. I assumed it was my ram so I have no replaced the ram and re-ran memtest. The good news: Memtest ran all the way to 100% with NO ERRORS (AWESOME!)
Bad News: Played Max Payne 3 for about 15 minutes....computer randomly restarted (No crashes, no BSOD). I'm hoping this was a fluke. Any ideas?

Note 1: Sig has been updated to show my new ram chips


Note: Video Drivers are up to date, and yes I tried disabling my anti-virus (Eset) as well

I'm not getting a crash to desktop, what's happening is the game is full on freezing in the game. When I alt+tab out I have the windows error log message going, but it can't find a solution. I've already torn apart my rig and put it back together to ensure its not overheating but it's still not better. This morning I got the strangest video glitch ever and I took a screenshot. Hopefully someone here knows a solution:

Have you determined whether your video card is the culprit? Whenever I had trouble with games freezing or having odd visual artifacting, it was always the card.

well I'm sure anything is possible. Is there an easy way to test this out?

Easiest way to test things is using a spare part that you know works.

Also, consider whether you've played any other resource intensive games on your PC. If those didn't mess with things, that may help absolve your video card.

Unfortunately I don't have a spare video card. The only other resource intensive game I've played recently is Diablo 3. Other than the standard server disconnects which most users have been having, it hasn't crashed or anything on me. The last game that gave me any real trouble that was similar to this experience was Battlefield 3.

I don't think Diablo III is resource intensive enough to really tax your card.

I'm certainly no expert, but my thought was that maybe some of your video card's memory is not working properly? If the memory module comprises some of the later address space (i.e. it would end up being filled last), most likely only resource intensive games would hit that problem.

If you saw issues with Battlefield 3, that might be an indication that the card is at fault. EVGA has a pretty good return policy, and might have Advance RMA options available, which means you would at least have a video card to use until the new one comes in.

So on another post I was given the suggestion of running tests for my CPU, GPU, and Memory. I was told to run Prime95 on my computer. I got my results and this is the report. I'm hoping someone can explain what this means and if this is the reason I'm getting these issues:
