A simple and cheap adapter takes care of HDMI. Not sure if it is, but MSI should included it with the card.

Powercolor just released a low profile 5750 too. Then you have the Palit low profile GTS 450. These are pretty much the best low profile cards out there, assuming your PSU is up to snuff.
