A little background:
Never bought a decent video card since I have always had a Xbox 360 / Xbox
OS X (10.6.4) didn't like my 7300 GS so I bought a BFG Tech GeForce 9800 GTX+ 1GB :D for $70

System Info:
Intel E8400 @ 4.2 Ghz
BFG Tech GeForce 9800 GTX+ 1GB
GIGABYTE GA-P35-DS3L
8 GB ram
x64 Win7
OS X 10.6.4
Display running @ 1440 x 900

Couple of things:
Holy crap this thing is loud! Well coming from a passively cooled 7300GS
Do I need to uninstall my old NVidia then install the latest one?
Any worth while tweaks?
After installing a game (Just installed Assassin's Creed II) do I need to change any in games video settings?
My GF's playing Assassin's Creed II on it and WOW  everything looks so AWESOME!

Thanks,

Skurge, perhaps you could explain fan profiles to the OP, maybe he has the fan at a high percentage, I didnt think they were that noisy?

Im not familiar with the nvidia control panel, so ill use rivertuner
http://downloads.guru3d.com/download.php?det=163

1. Install the driver, and Rivatuner first.
2. go to Rivatuner "Power User" page, and find "Rivatuner\Fan", click the + next to it
3. change value of "AutoFanSpeedControl" to 3 (this allows us to change the auto fan controller settings in the low level fan control window
4. go to "Low level system tweaks" page, "Fan" tab
5. now set the values for the auto fan control, don't forget to save it as a profile, and Apply Fan Settings at Windows Startup.

to calculate the values and what those value means, here's a explanation:
D min = minimum duty cycle
D max = maximum duty cycle
T min = minimum temperature (it's the lower limit when the graphics cards is operating, fan duty cycle will stick to Dmin when temp goes below this)
T rage = the range of temperature graphics card is expected to work at.
