Lost the link to the 15$ rebate for Sandisk 1GB cards at Outpost!

Last week, I picked up one of the SanDisk 1GB CF cards at Outpost for 55$ after a 15$ rebate. However, when I went back to get the rebate form after the memory arrived, they stopped showing it on the wepbage, despite the fact that the rebate should still work if you send it off in the next month or so. (http://shop1.outpost.com/product/3299811). Could someone provide me with a link, so I could get my rebate?
